I wish to participate in volunteer activities for Criacao Shinjuku (hereinafter referred to as "the Club"). I understand the purpose and content of the Japan Football League (hereinafter referred to as "the Tournament") and the Club's project (hereinafter referred to as "the Project"), and I pledge to strictly adhere to the following誓約事項 (pledge items). (Hereinafter, "Organizer" refers to the Club, the Tournament, or both, depending on the content of the Project.)

[Pledge Items]
1. I understand and will comply with all rules and regulations established by the Tournament and the Club, and I will follow the Club's instructions regarding all matters.
2. I will manage my own safety and physical condition and participate in volunteer activities at my own risk.
3. I confirm and agree that compensation for accidents and injuries during the Project will be limited to the scope of the insurance coverage purchased by the Club.
4. I will use any equipment provided for my participation in the Project carefully and will cooperate with setting up and tidying up. In the event of any damage to equipment, I will promptly report it to the Organizer.
5. I will manage my own valuables. I pledge not to hold the Organizer, Project-related personnel, Project participants, or general visitors responsible for any theft or loss.
6. I am not a member of an organized crime group or other anti-social force. I agree that my participation may be canceled if I am deemed to be an anti-social force.
7. I consent to the Club and its related parties freely using my photographs, videos, etc., related to the Project taken by them.
8. If I am a minor, I will participate with the consent of my guardian.
9. I agree that the personal information I provide for participation in volunteer activities may be used by the Organizer to the extent necessary for the Project.
10. I promise not to leak, disclose without legal permission, or privately use personal information of Project participants or related parties that I obtain through volunteer activities, nor will I abet such actions.
11. I will not take photos or videos related to the Project, or upload them to social media, etc., without the Club's permission.
12. I agree that any other issues not covered by the above items will be resolved by mutual agreement between myself and the Club.
