2026/27 JFL Matchday 1 Criacao Shinjuku vs Yokogawa Musashino FC "Escort Kids" Application Form

This form is for applying to be an "Escort Kid" at the match against Yokogawa Musashino FC on Sunday, August 30th. We apologize for the inconvenience, but please fill in the following information to apply.

22 people (If there are many applications, a lottery will be held.)

★Junior Member Priority Slot:
Criacao Shinjuku Membership "Junior Members" are given a priority lottery slot as a special benefit. If there are many applications, Junior Members will be given priority in the lottery.
*This does not guarantee selection.

[Day Schedule]

• Meeting Time: Around 18:00 (Kick-off at 18:30)
• Meeting Place: Designated location within Ajinomoto Field Nishigaoka
  *Details will be sent to selected participants via email.

[Eligibility]
Elementary school 1st graders to junior high school 3rd graders

[Activity Details]

• Enter the pitch holding hands with the players during player entrance.
• After entering, a commemorative photo session with the players is planned.

[Application Details]

• Application Deadline: Until 18:00 on Tuesday, August 25th (5 days before the match)
• Selection Announcement: You will be notified via email by Thursday, August 27th (3 days before the match).

[Attire and Belongings]

• Footwear: Since you will be entering the pitch (grass and track area), please be sure to wear athletic shoes (sneakers).
  *Participation in heels, sandals, leather shoes, cleats, etc., is not allowed.
• Apparel: For those who wish, T-shirts for escort kids will be lent on the day. Please return them after the event.

[Notes]

• Escort Target: In this event, only players from Criacao Shinjuku (home team) will be escorted. Away team players are not included. Also, please note that you cannot specify which player you will escort.
• Tickets: A ticket to watch this match is required to participate.
• Independent Participation: Limited to children who can participate independently by following staff instructions without guardian assistance.
• Other: If you are late for the meeting time, participation may be refused due to safety reasons.
• The content may be changed or canceled due to bad weather or operational reasons related to the match.
• Photos and videos of the event may be used on Criacao Shinjuku's official website, social media, and various other media. Please be aware of this beforehand.
